Format of request buffer:
Offset  Size    Description
00h    WORD    length of following data (max 13h)
02h    BYTE    11h (subfunction "Restore Erased File")
03h    BYTE    directory handle or 00h
04h    BYTE    length of volume name
05h  N BYTEs   volume name (including colon)
Note:   if both a directory handle and a volume name are specified, the volume
name overrides the handle

Format of reply buffer:

02h 15 BYTEs   ASCIZ name of erased file
11h 15 BYTEs   ASCIZ name under which file was restored
